*Position Overview: Embedded Advanced Case Manager:*
As an Embedded Advanced Case Manager, you'll play a pivotal role in coordinating comprehensive care services for patients, ensuring their healthcare needs are met effectively. Your expertise will contribute to improved patient outcomes, streamlined care, and optimal resource utilization.
*What You'll Need:*
* Excellent communication and interpersonal skills to collaborate with healthcare providers, staff, patients, and stakeholders.
* Basic understanding of hospital operations and steps needed to create a comprehensive discharge plan.
* Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care.
* Facilitating communication and information exchange between patients, healthcare providers, and other stakeholders involved in the care process.
* Monitoring patients' progress and adherence to care plans, providing support and education to promote self-management and preventive care.
* Conducting home visits or post-discharge to assess patients' living conditions, social support systems, and adherence to treatment plans.
* Advocating for patients' needs and rights within the healthcare system and community resources.
* Documenting patient interactions, care plans, and progress notes in electronic health records or care management systems.
* Participating in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration among healthcare professionals.
* Proficiency in healthcare management systems and technology, including electronic health records (EHR) and patient management software.
* Knowledge of healthcare regulations, accreditation standards, and compliance requirements.
* Commitment to patient-centered care and continuous quality improvement.
* Ability to build and maintain relationships with community organizations and stakeholders.
* Additional duties as assigned
*Qualifications:*
* Minimum of 2 years experience in a community mental health setting preferably with some experience in a hospital, crisis center, SMI clinic or behavioral health home
* Bachelor's degree in social service field (psychology, counseling, social work, public health) or a related field (or equivalent combination of education and experience)
