Located downtown in the One Call Building on the Baptist Medical Center of Jacksonville campus, we're seeking a passionate Physician with dedication for senior patients. We offer our physicians the opportunity to have a meaningful impact on the lives of the patients they serve without losing their way in the business of healthcare.

• Lead your care team, collaborating with experienced Advanced Practice Providers

• Support from an extensive in-practice team consisting of behavioral health, social work, pharmacy, nutrition, care coordination, and rehab resources .

• Locally governed, physician led, professionally managed and community focused program ensures autonomy and a voice in your practice

• Epic EHR

• M to F 8 am to 5pm

• On call 2 to 3 weeks per year for Monday through Sunday.

What You Will Need

• MD or DO

• Active/unrestricted medical license

• Board certified/eligible

• Family Medicine or Internal Medicine trained with a passion for senior care and population health.
