About the Role

This 13-week contract position is available to start as soon as possible, with a focus on working days. As a Case Manager RN, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare services.

Key Responsibilities

• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Closely monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
• Communicate effectively with patients, families, and healthcare providers to address concerns and coordinate care.
• Maintain accurate and detailed records of patient interactions and care activities.

Requirements

• Demonstrated expertise as a Registered Nurse with a strong background in case management.
• Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
• Ability to maintain confidentiality and handle sensitive information discreetly.
• Proficiency in electronic medical records (EMRs) and other relevant software applications.
