Home Health Physical Therapist Responsibilities:
- Evaluate & Treat Patients: Assess patients in their homes and develop personalized treatment plans to improve mobility, function, and safety.
- Adjust & Monitor Care Plans: Provide therapy, track progress, and modify interventions as needed to meet patient goals.
- Educate Patients & Caregivers: Teach patients and families exercises, mobility techniques, and fall prevention strategies.
- Supervise Support Staff: Oversee therapy assistants and home health aides to ensure quality care.
- Plan Safe Discharge: Develop discharge plans, provide follow-up resources, and train caregivers as needed.
- Ensure Compliance: Maintain accurate documentation and meet insurance and regulatory requirements.
- Manage Pain: Conduct pain assessments and implement appropriate interventions.
- Support Team & Operations: Perform additional tasks to enhance patient care and agency operations.
Physical Therapist Qualifications
-Experience: 2+ years of experience (required); 1+ year in Home Health (strongly preferred)
