  • COVID-19 Vaccine - 2 COVID-19 vaccines and booster required or declination completed
  • MMR - Proof of 2 Vaccines or Positive Titer showing immunity
  • Fit Test - Annual Respirator fit test.
  • Hepatitis B - Declinations not accepted at Shasta Regional Medical Center.
  • Influenza Vaccine - Flu Vaccine or declination required October1-May 31. Declinations not accepted at Shasta Regional Medical Center.
  • TB Test - Required.
  • Varicella - Vaccination/titer showing immunity
  • Tdap - Declinations not accepted at Shasta Regional Medical Center.
  • Annual TB Questionnaire - Required.
  • Physical/Health Statement - Annually, Except Saint Marys Regional Medical Center- Nevada must be completed within 60 days of start. Must state clear to work.

  • National Criminal Background Check within one year of start date.
