  • Prime Drug Screening (minimum 10-panel test) within 60 days of start.
  • Prime Color-blind test (must be completed annually).
  • License/Certificate Verification (Primary Source Verification of professional license, and/or certification, as required for the position. Ran within 30 days of start. Updated upon expiration.)
  • COVID-19 Vaccine (2 vaccines and booster required or declination completed).
  • MMR (Proof of 2 Vaccines or Positive Titer showing immunity).
  • Fit Test (Annual Respirator fit test).
  • Hepatitis B (Declinations not accepted).
  • Influenza Vaccine (Flu Vaccine or declination required October 1-May 31).
  • TB Test (Annual TB Questionnaire).
  • Varicella (Vaccination/titer showing immunity).
  • Tdap (Declinations not accepted).
  • Physical/Health Statement (Annually, Except Saint Marys Regional Medical Center- Nevada must be completed within 60 days of start).
  • Prime Core Competencies Exam (Must score 80% or higher).
  • Prime Criminal Background Check (National Criminal Background Check within one year of start date).
  • Prime Badge Photo (JPEG only photo).
  • Prime Form I-9 (Employment eligibility verification).
  • Prime Job Description (Signed by employee).
  • Prime COA.
    Schedule Information
  • Rotating weekends, must be able to do holiday coverage.
