In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes!
You will be primarily in the home health setting with dedicated time and caseload support for the SNF. Rivetus has been serving the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings.

Schedule: Monday - Friday, Full Time
Successful completion of an accredited Occupational Therapy program.
Ability to react effectively to the needs of patients and their families.
