Responsibilities: - Regular nurse visits
- Wound care
- Develop individualized care plans
- Administer medications and treatments as prescribed
- Educate patients and their families on managing illnesses and promoting wellness
- Collaborate with interdisciplinary teams to ensure coordinated and effective patient care
- Monitor and document patient progress and make appropriate recommendations
Qualifications: - Active RN license in Florida
- Minimum 1 year of clinical experience, preferably in home health or a related field
- Proficient in clinical assessments and care planning
- Strong communication and interpersonal skills
- Ability to work independently and within a team environment
- Reliable transportation and valid driver's license
Preferred Qualifications: - Certification in Home Health Care
- Experience with electronic medical records (EMR) systems
