- This is a salaried full-time, Monday through Friday, Daytime position.
- This position requires valid Pennsylvania CRNP and Registered Nurse licenses, a valid Pennsylvania driver's license, and reliable internet access.

As a Nurse Practitioner you would be responsible for performing patient medical examinations and palliative care visits relative to the face-to-face encounters required to certify patient's eligibility for the Medicare Hospice Benefit, after 6 months of hospice care. You would document visits in the patient's clinical record and face-to-face encounter on required forms. You would provide medical care services and palliative care intervention for hospice patients on an as-needed basis. This position works in coordination with the Medical Director, Hospice Director, the Hospice Interdisciplinary Team, and other Nurse Practitioners.
Requirements: Education
- Nurse Practitioners must have a master's degree or higher to qualify for certification in Pennsylvania.
- Education includes advanced studies in anatomy, physiology, chemistry, psychology, nursing and microbiology. Master's programs include a combination of classroom and clinical studies. Master's nursing students study advanced health assessments to prepare for the diagnosis and evaluation duties of a practice nurse.
- National certification requires the candidate to successfully pass an examination.
Experience
- Minimum of two (2) years' experience as a Nurse Practitioner.
- Must have understanding of the palliative/hospice program of care.
Licensure, Registration, and/or Certification
- Currently licensed and nationally certified as a nurse practitioner per the Commonwealth of Pennsylvania's Board of Nursing (CRNP)
- The candidate must have submitted proof of 45 hours of advanced pharmacology education. The course must have been part of an advanced program and not pharmacology courses that are part of the registered nursing program.
- Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania requirements.
- Satisfactory criminal background checks.
- Must have reliable internet access.
