Job Description

Opened in the Spring of 2020, The Lakes of Clermont Health & Rehabilitation Center is conveniently located in the southern half of Lake County. We are an 80-bed, fast-paced, skilled nursing community full of dedicated and compassionate caregivers, and we are looking for additional team members to work with our at-risk patient population.
Our community is seeking a candidate for our MDS RN opening.
This role will:

  • Oversee and coordinate the development and completion of the member assessment (MDS) in accordance with current Federal and state regulations.
  • Coordinate and lead the development of a comprehensive member assessment and care plan for each member as chairperson of the community Interdisciplinary Plan of Care team.
  • Ensure the face validity of all Minimum Data Sets before electronic submission and combine the timely electronic submission of all MDS.
  • Conduct a monthly review of a random sample of the MDS to ensure accuracy and internal consistency with the medical records.

Qualifications:
  • Must be an active Registered Nurse (RN) in good standing with a viable license in our state.
  • Must have at least 2 years' experience as a Nurse.
  • Knowledge of PPS assessment as well as OBRA compliance.
  • Demonstrates clinical assessment skills at the level necessary to meet the job requirements.
  • Ability to make independent decisions and follow through with limited supervision.
  • Knowledgeable and understanding of state and federal regulations and guidelines governing the practices of the facility.
  • Effective interpersonal skills and the ability to work cooperatively with an interdisciplinary care team.
  • Able to relate positively, favorably, and cooperatively with others, including, members, family members, associates, and personnel of outside agencies and organizations.
  • We are a vaccinated care facility. New hires will need to provide proof of vaccination or other acceptable documents prior to starting employment.
