Responsibilities:n

• Conducted health assessments and created personalized care plans for each resident.n
• Administer medications and treatments according to physician orders and established protocols.n
• Monitor residents' vital signs and promptly address any changes in health status.n
• Collaborate with interdisciplinary teams to provide holistic and person-centered care.n
• Assist in the development and implementation of wellness programs and activities.n
• Educate residents and their families on health-related topics and promote preventive care.n
• Respond to medical emergencies and provide immediate and appropriate interventions.n
• Maintain accurate and up-to-date resident medical records.n
• Foster a supportive and compassionate environment for residents and their families.n
• Participate in ongoing training and professional development opportunities.n

Requirements

You love working with people and handle everyday challenges with a smile!

• Graduate or Future Graduate from an accredited nursing programn
• Hold a Current State License as a Licensed Practical Nursen
• Able to communicate well in both verbal and written formsn
• Have a minimum of 1 year experience as an LPN in a long term care settingn
• A natural problem solvern
• Are committed to interacting professionally and with care toward residents, families, and other staffn
