- Need Certified Nurse Practitioners for Home Health Care- Primary Care
- Monday- Friday
- minimum 60 days
- Consults/ Follow up visits/ Annual wellness
- Flexible scheduling: Candidate can schedule their own days, or utilize administration to do scheduling. Expectation is onsite min. 4 homes/ day.
- Each visit is approx 1.5hrs
- Mileage provided
- Scope of patients: Stage 3 kidney failure
- Must have active state license
- Athena EMR along with Salesforce
- Nephrologist on staff available to patients
- Credentialing 45-60 days
- Training program in Nashville (paid travel/lodging) or can be done virtually. This can be done while in credentialing. Training is held every 2 weeks.
