Position Highlights
  • Schedule : Monday - Friday, day shifts only
  • Limited on-call coverage (phone-based only)
  • We can assist with obtaining a Virginia Medical License

Overview:
Are you a dedicated and caring Nurse Practitioner (NP) looking to make a meaningful impact in long-term care ? Join our growing team to deliver exceptional patient care coordination and support geriatric populations in your community. This full-time opportunity offers the chance to work in a dynamic medical practice, with resources and training to ensure your success as an Advanced Practice Nurse specializing in geriatric nursing .
Key Responsibilities
  • Provide high-quality, compassionate care for patients in long-term care settings , focusing on geriatric nursing
  • Collaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.
  • Accurately document visits in an electronic medical record (EMR)
  • Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse .
Qualifications
Required Qualifications:
  • Active Virginia Medical License and DEA License
  • Minimum one year of clinical nursing experience
  • Strong communication skills, self-motivated, adaptable, and a team player
Preferred Qualifications:
  • Nursing experience in ICU, Med/Surg, Emergency Medicine , or Hospitalist roles
  • Experience with care coordination and working in long-term care settings, especially geriatric nursing
