DescriptionnnAt Western Connecticut Home Care, we are seeking individuals to become part of our exceptional home healthcare team. As part of the Nuvance Health Network, our home health staff enjoy flexible schedules; continuing education options; excellent medical, dental and recreational benefits; and the support of a thorough orientation program.nnSummary:nnProvides services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain with emphasis on upper extremity function, maximal independence in activities of daily living, and cognitive perceptual motor function. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any) and caregiver. Communicates with the physician who is responsible for the health plan of care and other health care practitioners as appropriate related to the current home health plan of care.nnResponsibilities:nnDevelops, implements and evaluates te plan of care in partnership wi the patient, representative (if any), caregiver(s) an physician(s).nnProvides services that are ordered by the physician as indicated in the plan of care. Communicates with all physicians involved in the plan of care and other health care practitioners as appropriatennProvides direct occupational therapy services in accordance with the plan of care established by the physician.nnMaintains records on visits to clients; including evaluation, plans for care, client's response to therapy. Provides discharge planning when applicable, and clinical notes.nnParticipates in case management conferences with the nurse and other team members to coordinate the occupational therapy program with total plan of care.nnProvides education and counseling to the patient and family members regarding occupational therapy treatment regimen.nnIdentifies patient and family needs for other home health services and makes client referrals for same, when needed.nnParticipates in the agency quality assurance and performance improvement program and the agency-sponsored inservice training.nnSupervises care provided by the COTA(s) and home health aides as appropriate..nnRe-evaluates the client's progress and treatment regime, making changes as indicated, and under the physician's recommendations.nnFulfills all compliance responsibilities related to the position.including in-service training.nnPerforms other duties as assigned.nnOther information:nnRequired:nnMinimum Experience: one yearnnDesired: Experience in qualified home health agency.nnLocation: Danbury-100 Saw Mill RoadnnWork Type: Per DiemnnStandard Hours: 0.01nnFTE: 0.010000nnWork Schedule: Day 8nnWork Shift: VariablennOrg Unit: 610nnDepartment: Occupational TherapynnExempt: YesnnGrade: S11nnSalary Range:nn$32.6400 - $65.0100 HourlynnWorking conditions:nnEssential:nnSignificant manual skills / motor coord & finger dexteritynnVery Heavy effort.
