Position Title: Behavioral Health Therapist
Location: Upland Hills Health - Dodgeville Medical Center
Role & Department: Behavioral Health in the Behavioral Health Clinic
Hours & Shift: Part-time to Full-time (.5 to a 1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.
Role Responsibilities:
The Behavioral Health Therapist is responsible for providing initial evaluations, individual, group, couples and/or family therapy at the clinic
Will provide behavioral health counseling services and conduct patient assessment and establish therapy treatment goals
Will maintain thorough and confidential records of therapy sessions
Therapist will work collaboratively with the primary care providers on treatment plans in the ambulatory setting
Act as advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse
Will assist other care coordination team members by providing behavioral health services to patients and their families in the outpatient setting to enhance psychosocial functioning and continuity of care
Provides advocacy, information and referral services to patients and families to address their psychosocial needs
Provides assistance with overall program development, community development, training, and education in this specialty area
Participates in continuous quality improvement activities and educational experiences to support organizational objectives and high-quality care
Conducts regular appointments with clients, including diagnostic assessments, therapy, and care management services
Will establish a positive and trusting relationship with clients
Will continuously assess clients and re-evaluate treatment plans as appropriate
Will refer clients to outside mental health support and/or agencies as appropriate
Will maintain privacy and confidentiality of each client and their situation
Will record, maintain, and sign documentation according to standards outlined by best patient practices in this specialty
Will manage client load and schedule, including appropriate correspondence and documentation to support excellent client care
Will implement outlined rapid triage and assessment algorithms to provide clients with appropriate treatment plans, as is deemed necessary
Qualifications:
Requires a Master’s degree as an MSW
LCSW license required or in the process of obtaining
MSW-APSW's will be considered
Two years of experience providing clinical services in a mental health capacity is preferred
Two years’ experience in individual and/or group therapy services preferred
Candidates must have wonderful interpersonal skills and ability to work with patients, physicians, families, managers, and members of the community under varied conditions
Excellent organizational, critical-thinking, communication and conflict resolution skills required
Tolerance of stress and the ability to recognize your own limitations and respond to difficult situations
The ability to apply your knowledge and research to clinical problems
The capacity to be critical and analytical with patients when necessary
