*Job Title:* Home Health Physical Therapist (Contract - W2)

*Location:* Los Banos, CA

*Employment Type:* 13-Week Contract

*Schedule:*
* Days, 40 hours per week
* No on-call requirements
* Occasionally will need to work 1 weekend a month or 2 Saturdays / Sundays a month

*Pay:*
* Estimated Gross Weekly Pay: $3,400-$3,446/week (depending on lodging and per diem eligibility)

*Key Responsibilities:*
* Provide physical therapy services to home health patients, with an average of 4 patients per day.
* Work with a supportive team and receive peer-to-peer mentorship throughout the contract.
* Conduct assessments, develop treatment plans, and provide therapeutic interventions.
* Common cases include orthopedics, wound care, disease education, medication management, wound vac, and hospital-at-home.
* Collaborate with other healthcare professionals to provide comprehensive patient care.

*Qualifications:*
* *License:* Active CA Physical Therapy License required.
* *Experience:* Minimum 2 years of experience in Physical Therapy. Hospital Critical Care or Home Health experience is preferred.
* *Certifications:* BLS Certification required.
* *Skills:* Must be a go-getter with a willingness to learn and help. Strong communication skills and the ability to work independently in a home health setting.
* *Personality:* A proactive, adaptable personality is key for success in this role.

*Training:*
* Comprehensive training on Oasis and Homecare Homebase EMR systems (20 hours over 4 weeks).
* Pairing with a peer mentor for continuous support and learning throughout the contract.
