We are seeking a Group Home Nurse (RN) Supervisor to oversee our Norwich, CT home of individuals with Developmental and Intellectual Disabilities.
The Group Home Nurse Supervisor works 40 hours/week, Monday-Friday 8a-4p and ensures the provision of all health services for program participants by overseeing the job performance of the agency RNs and SLP. The Group Home Nurse Supervisor is responsible for coordinating all contracted health care related services, such as PT, OT and any additional services required to ensure the well-being and optimal care is delivered. This position will also ensures DDS, DPH and Nursing Standards of Practice are consistently demonstrated and maintained.

* Current Connecticut Registered Nurse license
* BSN
* Professional nursing experience
* Valid driver’s license:
* Knowledge of health and medical needs of individuals with disabilities; ability to plan and implement ways of meeting these needs in a community setting.
* Knowledge, skills, and ability to assess and document the health needs of participants. Ability to organize and maintain accurate records.
*Responsibilities *
* Works in conjunction with the Group Home Nursing Coordinator and Planning and Support Team and collaborates on setting standards and creating policies for nursing services across the agency.
* Holds monthly meetings and training courses with agencies RN Coordinators. Completes job performance reviews of RN Coordinators. Addresses any RN Coordinator performance concerns in conjunction with the Director of Nursing.
* Works with RN Supervisors to ensure on call after hours schedule is fair and rotates through all RNs, as applicable.
* Provide on call coverage on a rotating basis with other RN Supervisors, as indicated by the agency on-call policy and procedures
* Approves RN Coordinator case load changes to meet the needs of the locations and cover all vacancies.
* Ensures that each RN Coordinator secures medical, dental, pharmacy and related health care services in the community, and works with health professionals maintaining regular contact, also works with families and vendors, to remain informed of changes which might affect services for participants.
* Works closely with the Director of Behavioral Services and Senior Director to ensure consultant psychiatrists and other health care professionals are providing necessary services, to ensure the well-being of participants and is in conformance with the DDS and DPH standards and regulations.
* Ensures that RN Coordinators communicate problems and issues relating to the health of participant’s and follows up in writing with timely documentation supported by the agency and funding source requirements.
* Attends DDS regional meetings and encourages RN Coordinators to participate
* Teaches new nurses and ensures that RN Coordinators fully utilize Electronic Health Records, to review, document and monitor the participant’s health. Ensures RNs create quarterly and annual health care summaries and submits these to the Planning and Support Team prior to IP/OPS meetings, to ensure input on health-related issues. Reports communicable diseases to the RN supervisor so that proper prevention measures can be devised and implemented.
* Teaches Medication Certification refresher classes and teaches health related issues, including safe storage, handling and administration of medications; health care needs, and personal care / hygiene; in order that they have the information and delegation required to ensure that participants maintain good health.
* Monitors medication administration records, incident reporting, restraint and other health related reports, and follows ups, as warranted, to ensure compliance to funding source requirements.
