  • Job Summary: RN OR position at Gottlieb Memorial Hospital, Melrose Park, Illinois.
  • Experience with EPIC and PYXIS access preferred.
  • Schedule: 0700-1900 OR 0700-1700 Mon-Fri; call Sat/Sun prn.
    Job Requirements
  • BLS certification required for all direct patient care personnel.
  • License/Certificate Verification must be dated within 30 days of start.
  • Fit Test required.
  • Trinity 9 Panel Drug Screen must be completed within 30 days of start date.
  • Trinity Code of Conduct Form must be signed and on file.
  • Trinity I9 must be completed by the agency.
  • Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed and on file.
  • Trinity Specialty Specific Competency Testing must be completed within a year.
  • 2 Step PPD OR IGRA (Quantiferon) required.
  • COVID-19 Vaccine proof required.
  • MMR vaccination proof required.
  • Hepatitis B vaccination proof required.
  • Influenza Vaccine proof required.
  • Varicella vaccination proof required.
  • Tdap vaccination proof required.
  • Physical/Health Statement required.
    Schedule Information
  • 0700-1900 OR 0700-1700 Mon-Fri; call Sat/Sun prn.
    Unit Specific Information
  • Experience with EPIC and PYXIS access preferred.
    Additional Details
  • American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS.
