Join our compassionate team dedicated to providing exceptional care to patients in their homes. We’re seeking a skilled and empathetic Medical Social Worker to support our client in navigating the challenges of illness and medical care.

Key Responsibilities:

Conduct assessments to identify clients' psychological, social, and medical needs.
Collaborate with clients and families to develop and adjust personalized care plans.
Connect clients with community resources to enhance their well-being.
Provide support during discharge planning and case conferences.
Maintain accurate and timely documentation in line with agency policies.
Supervise social work assistants as needed and ensure the delivery of quality care.
Performs other duties as assigned

Qualifications:

Graduate of an accredited college with a Master of Social Work degree or equivalent
Current state licensure.
Minimum one year of supervised or equivalent work experience (home care experience preferred).
Strong communication, organizational, and problem-solving skills.
