Department: Center for Women and Children

Reports To:  Family Ministry Coordinator

Classification:  Full time/ non-exempt

Position Overview:  The Case Manager at the Center for Women and Children is responsible for providing comprehensive case management services to women who are residents or clients of the CWC. The role involves assessing needs, developing personalized service plans, and connecting clients to essential resources and support systems. The Case Manager will work collaboratively with clients to empower them, ensuring they have access to services that promote safety, stability, and well-being.

Key Responsibilities:

  • Client Assessment and Intake:  Conduct intake assessments and comprehensive needs evaluations for women and children seeking support. Collect relevant information to identify the physical, emotional, social, and economic needs of each client.
  • Individualized Service Plans:  Develop and implement personalized case plans in collaboration with clients, outlining goals and strategies for achieving self-sufficiency and stability.
  • Resource Coordination:  Connect clients with appropriate community resources, including housing assistance, employment support, legal services, healthcare, counseling, childcare, and financial aid.
  • Advocacy:  Serve as an advocate for clients in navigating complex systems, ensuring they understand their rights, and supporting them in accessing available services.
  • Regular Check-ins and Support:  Monitor client progress through regular check-ins and case management meetings. Adjust case plans as needed to address evolving needs or challenges.
  • Crisis Intervention:  Provide immediate assistance and support to clients experiencing crises, including emotional distress, housing instability, or other emergencies. Coordinate with internal and external partners for urgent intervention.
  • Documentation and Reporting:  Maintain accurate, up-to-date records for all cases, including case notes, progress reports, and service plan updates. Ensure compliance with privacy regulations and organizational policies.
  • Education and Empowerment:  Educate clients on topics such as financial literacy, self-advocacy, legal rights, and other life skills. Empower clients to take an active role in their own care and decision-making.

Qualifications:

  • Minimum of 2 years of experience in case management or a social services role, particularly working with vulnerable populations.
  • Knowledge of community resources and services, including housing, mental health, healthcare, employment, and legal aid.
  • Strong communication, interpersonal, and active listening skills.
  • Ability to assess situations, make sound decisions, and provide appropriate interventions.
  • Compassionate, client-centered approach to case management with a focus on empowerment and long-term stability.
  • Strong organizational skills with attention to detail and the ability to manage multiple cases.
  • Experience in crisis management and de-escalation techniques.
  • Ability to maintain confidentiality and adhere to ethical standards in case management.
