*Job Title:* Home Health Physical Therapist (Contract - W2)

*Schedule:*
* Days, 40 hours per week
* No on-call requirements
* Occasional mandatory office visits (typically Tuesdays, Wednesdays, or Thursdays)

*Pay:*
* Estimated Gross Weekly Pay: $2,400-$2,506/week (depending on lodging and per diem eligibility)
* Mileage reimbursement: $0.67/mile

*Key Responsibilities:*
* Provide skilled physical therapy services in a home health setting across Denton, Grayson, Krum, and Aubrey areas.
* Complete documentation using Homecare Homebase (HCHB) at the patient's home.
* Manage a caseload of 20-25 cases per week, achieving a daily productivity goal of 26-28 points.
* Deliver high-quality care, ensuring all documentation is completed on the same day.

*Requirements:*
* Active Physical Therapist (PT) License in Texas.
* Minimum of 2 years of recent home health experience.
* Proficiency in OASIS-E documentation (experience after January 2023 is required).
* No gaps in employment exceeding one year.
* Basic Life Support (BLS) certification.

*Orientation:*
* 2-3 days, including online modules and team introductions.

*Other Details:*
* Scrubs: Any color.
* Flexible schedule required to accommodate patient needs.
