764A CARE COORDINATION- Social Worker

Min of 2 years of experience within specialty - Hospital Based experience.
Required: BLS & LCSW required in the state of GA. Masters of Social Work (MSW) OR B.S. degree in related field with strong clinical background. Two (2) years previous experience in case management, utilization review, or social services. Ability to interact with patients, families, physicians and hospital staff. Excellent oral and written communication skills. Ability to type accurately at an approximate rate of 30 wpm
