Job Description
  • Coverage Needed:
    • April 28, 2025 - ongoing
    • Cover 2 facilities
    • 24 hour shifts
    • Specific Dates:
      • April 28-29
      • June 23-27
      • August 4-8
  • Clinical Information:
    • Responsibilities: Hospital rounds, emergency surgeries
    • EMR: EPIC
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IN License and DEA (or easily transferrable due to need)
    • Board Certified
