Family Medicine to join their team! This assignment offers
Monday Friday, 7:30 AM 4:30 PM daily clinic coverage,
no weekend coverage, and
possible clinic call.

Patients per day: 20
Provide information on support staff, APP coverage, number of providers in the clinic and on-site services (lab, radiology, procedure room:)
Support Staff = MA and Receptionist, POC testing Diagnostic Imaging in building
Current EHR in use: Healthland
Family Medicine Procedures/SkillsAdult Outpatient
Pediatric-General Outpatient
Please note:Will only accept Board Certified Submission Requirements- Board Certified(Family Medicine)
- Provider must be local
- Must have clean background for quick credentialing
- DEA
- MI License
- BLS
Employee Health Requirementa. Flu vaccine for 24-25 season
b. Covid documentation - If 24-25 booster has not been received a declination is needed
c. TB - 2 step skin test within 12 months or Quantiferon within the last 6 months
d. TB Questionnaire
e. Hep B - titer with quantifiable numbers or declination
f. MMR - vaccine series or titers with quantifiable numbers
g. Varicella - vaccine series or titers with quantifiable numbers
h. TDAP - adult vaccine (within the last 10 years)
