- Position Specific Performance Expectations:*
- Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with interdisciplinary team.
- Restores, maintains, and promotes not only optimal physical function but optimal wellness and fitness and optimal quality of life as it relates to movement and health.
- Prevents the onset, symptoms, and progressions of impairments, functional limitations, and disabilities that may result from diseases, disorders, conditions, or injuries.
- Recommends use of assistive devices and provides gait training, as deemed necessary.
- Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; Documents patient care services via EMR charting using 'defensible documentation' in a timely and concise manner, in compliance with industry standards.
- Protects patients and employees by adhering to infection-control policies and protocols.
- establishes personal networks; Develops rehab and hospital staff by providing information; developing and conducting in-service training programs.
- Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; Strives to promote health, creates a safe and effective treatment environment, motivates others, has a good bedside manner, listens, analyzes information, and is quality focused.
- This includes seeking opportunities to gain new knowledge and skills and utilizing these newly acquired skills in the provision of care and for growth and development of the department and hospital.
- Critically assesses self to improve clinical and professional performance.
- Advocates for the physical therapy profession.
- Collaborates with the school-based team to help determine and carryout Individualized Education Programs (IEPs); Home health physical therapists (in addition to above expectations):*
- Provide in-home physical therapy services to homebound patients and works in collaboration with the home health interdisciplinary team.
- Organization Wide Performance Expectations:*
- Establishes and maintains effective verbal and written communication and good working relationships with all patients, staff and vendors.
- Adheres to MRH attire/dress code per policies and procedures.
- Completes annual education, training, in-service, and licensure/certification requirements; attends departmental and organizational staff meetings or reads meeting minutes.
- Actively participates in departmental and facility performance improvement and continuous quality improvement activities.
- Strives to uphold regulatory requirements to ensure continual compliance with departmental, hospital, state and federal regulations and policies.
- Follows policies and procedures for infection control, safety, and risk management to ensure a safe environment for patients, public, and staff.
- Must complete employee health meeting.
- Active Colorado Physical Therapy License (DORA) or compact license.
- Current CPR (BLS) certification (within 90 days of hire per MRH Policy).
