JOB SUMMARY:

• This role involves conducting medical management and health education programs for individuals enrolled in government healthcare programs.
• The position requires performing assessments, creating care plans, and coordinating care both telephonically and in the member's home.
• Remote – but must live near Raton, NM

RESPONSIBILITIES:

• Conduct medical management and health education programs for government healthcare program members.
• Perform assessments, develop care plans, and coordinate care in member/family homes and via telephone.
• Collect, analyze, and report data to meet regulatory requirements.
• Serve as the primary company representative for members, fostering a positive relationship and addressing their healthcare needs.

REQUIRED EXPERIENCE:

• Registered Nurse (RN), Licensed Professional Counselor (LPC), Licensed Master Social Worker (LMSW), with 3 years direct clinical care to the consumer in a clinical setting.
