Experience: 1 year of experience as an LPN, particularly in an LTC facility
License: NY LPN Licensure
Certifications: BLS
Must-Have: Ability to provide care to geriatric residents
Job Description:
- Demonstrate the knowledge and skills necessary to provide age-appropriate care for adult and geriatric patients.
- Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care, involving the resident/family and the healthcare team in the process, and promoting a healthy, safe environment.
- Recommend revisions to the care plan after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
- Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice, ensuring customer satisfaction.
- Maintain current knowledge and competency in Nursing Practice. Participate in mentoring/orientation of staff. Consider factors related to safety, cost, and impact in planning the delivery of care.
- Under the direction of the RN, provide leadership in the professional practice setting. Demonstrate knowledge and competency.
- Anticipate occupational exposure to bloodborne pathogens. Tasks may involve potential contact with blood, body fluids, or tissues.
