Job Description

The Occupational Therapist is responsible for performing client evaluations, developing and providing occupational therapy services, and documenting services in accordance with the plan of care developed for each patient.nnEssential Duties and Responsibilities:nnProvides skilled occupational therapy services/interventions in accordance with physician ordersnnReevaluates client for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriatennAssesses patient needs, plans for, evaluates and modifies care to meet goals of Occupational Therapy InterventionsnnCollaborates with all disciplines to plan and evaluate team goals for each patientnnInstructs patient/family/caregivers on patient's total occupational therapy programnnAdvises, consults and instructs members of the multi-disciplinary team in specific occupational therapy modalitiesnnObserves and reports patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team membersnnRefers individual to other disciplines as deemed appropriate and as approved by the physiciannnTeaches new skills or retrains clients in once-familiar daily activities that have become difficult or unfamiliar due to illness or injurynnEstablishes equipment needs to facilitate independent livingnnInstructs individual and/or family/caregiver in the use of various types of adaptive equipment to improve function; ensures equipment is in good working ordernnTrains individual and/or family/caregiver in use of prosthetic and/or orthotic devicesnnPerforms work capacity evaluations and may refer individual to specialized vocational training in accordance with policy and regulationsnnProvides accurate and timely documentation according to the plan of carennParticipates in care conferences or other meetings as required to ensure coordination of home health services (if applicable)nnPrepares and submits clinical progress-summary based on the progress of goalsnnProvides in-services/education programs for various disciplines as needednnParticipates in peer consultation processnnSupervises Occupational Therapy Assistants according to company policies/procedures and state regulationsnnParticipates in discharge planning when applicablennComplies with applicable local, state, and federal laws and regulations regarding professional standards and practice standardsnnMay fabricate splints and instruct individual and/or family/caregiver in the use, application and benefitsnnPerforms other duties as assigned/necessarynnMinimum Requirements:nnCurrent licensure as an Occupational Therapist in the State of practicennMust be a graduate of an accredited school or program of Occupational TherapynnOne (1) year of prior professional Occupational Therapy experience preferrednnComplies with all relevant professional standards of practicennParticipation and completion of Amergis' Competency program when applicablennCurrent CPR if applicablennTB Questionnaire, PPD or chest x-ray if applicablennCurrent Health certificate (per contract or state regulation)nnMust meet all federal, state and local requirementsnnSuccessful completion of new hire training as applicable to job sitennUnderstand patient confidentiality and HIPAA requirementsnnAbility to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is requirednnComputer proficiency requirednnMust be at least 18 years of agennBenefitsnnAt Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits:nnCompetitive pay & weekly paychecksnnHealth, dental, vision, and life insurancenn401(k) savings plannnAwards and recognition programsnn*Benefit eligibility is dependent on employment status.nnAbout AmergisnnAmergis, formerly known as Maxim Healthcare Staffing, has served our clients and communities by connecting people to the work that matters since 1988.
