A home care organization in Rhode Island is seeking a LPN to join their team!
About the Opportunity:

• Schedule: Monday to Friday
• Hours: 8am to 5pm
• Patient Population: Geriatrics
• Patient Volume: 6 to 8 per day
• EMR: ECW

The LPN will be responsible for:
- Assist in developing and implementing treatment plans under clinical supervision, actively participating in team meetings.
- Provide clinical services in various settings, including co-visits, telehealth, and independent visits.
- Deliver clinical services as directed by patient assessments and care plans.
- Conduct health screenings for new and ongoing patients.
- Perform field clinical tasks such as wound care, phlebotomy, and medication review.
- Re-engage unreachable patients in their homes.
- Offer coverage for team members' absences to ensure uninterrupted care.
- Collaborate with the team to enhance care quality and patient/staff satisfaction.
Qualifications:
- Licensed Practical Nurse in the state of employment.
- Experience in holistic care, including mental health and substance use.
- Current CPR and BLS certification.
- Proficient in Microsoft Word, Excel, and Outlook.
- Ability to travel efficiently, with proof of vehicle insurance.
- Driver requirements: Licensed for 5+ years with no recent infractions or convictions.
