Job Description Find your purpose as a Licensed Practical Nurse at CentraCare Northway Clinic. This clinic features Integrated Behavioral Health Services which gives patient the opportunity to access these services in a family practice setting. The LPN will provide patient focused customer service to all individuals, oversee and coordinate preventative care services and community resources for patients and their families to achieve good health, help patients achieve optimal health by planning patients care using evidenced based guidelines for care, (i.e., Hgb A1C or immunizations as appropriate) or gathering information pertinent to the patients care from other sources and assist in assuring the day-to-day activities of the clinic are completed timely and efficiently which includes rooming patients.
Schedule: - Casual
- Various shifts as needed Monday-Friday between 7 a.m. and 5 p.m.
- No evenings, weekends, or holidays
Pay: - Pay range $20.20 to $30.30 depending on years of experience.
Licensed Practical Nurse: - Graduate of an accredited school of nursing required.
- Current licensure as a Licensed Practical Nurse (LPN) in the State of Minnesota required.
- Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
- Ability to understand patients' medical records, physician orders and medication orders to be communicated to the patient and staff.
