Please note: We are hiring for either two part-time positions each at 20 hrs/wk or one full-time position at 30-40 hrs/wk. Position Summary This position works with a medical team and/or an outreach worker to provide street-based medical care to people experiencing homelessness. This generally includes assisting with assessing the patient, documentation, and administering treatment. LPNs will also provide phlebotomy in a clinic setting as well as in an outreach setting. This position will work with the provider and other clinic staff to deliver compassionate, quality care. Essential Functions & Responsibilities Provides appropriate medical services and medical support services, including information, injections, lab coordination, phlebotomy, pharmacy coordination, vitals, and coordination with both psychiatric and medical providers Implements medical plan through administering medications in accordance with Health Care Practitioner's orders and protocols Implements medical plans through obtaining diagnostic tests in accordance with Health Care Practitioner's orders and protocols: Obtains and processes body fluid specimens and performs EKGs using proper techniques and procedure Communicates information to internal and external partners using established referral process Uses pharmacy knowledge and available resources to include drug reaction and overdose in administration of medications Assists in planning an individual treatment program by using available resources in planning care, and consults with NP and other clinical staff as appropriate while applying knowledge and resources in planning care and patient teaching Document patient information, condition, and treatment Assist Provider in scribing, documentation, and follow up Maintain patient confidentiality and perform all care with respect for patient rights Maintain a safe, clean, and well-stocked environment for patients Operate and drive agency vehicles in conjunction with applicable safety policies and laws Participate in required training & meetings Other duties as assigned Required Qualifications High school diploma or general education degree (GED) required LPN License for state of Oregon BLS Certification (Required) Previous medical back office experience/medical assistant training, or completion of a MA/CMA/LPN program preferred. Ability to complete all essential functions related to LPN scope Ability to identify and respond to issues of confidentiality. Knowledge of HIPAA. Ability to utilize and apply medical terminology appropriately. Valid Oregon Driver's license, or valid out of state driver's license with the ability to get an Oregon Driver's license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy Ability to follow oral and written instructions Demonstrated ability to implement clinical care in accordance with outlined policies and procedures For a complete job description, please visit:
