We provide priority vaccine access, a ready supply of quality PPE, and up-to-date training.nnPosition Overview: Responsible for supervising the personal care, companion and skillednnnursing services provided by company’s direct care staff; coordinating clinical services, andnnensuring quality of care of clients in assigned area. Duties include performing in-the-homennassessments to determine support and clinical needs, conducting regularly schedulednnsupervisory visits of clients, documenting visits, and orientation and training of caregivers asnnneeded. Performance of skilled tasks may be required.nnResponsibilities:nnEnsures that initial assessment and reassessment (supervisory visits) are performed timely asnnrequired by rules and regulations.nnSupervises and evaluates the reassessment (within ten days) and skilled work performed bynnLPNs as defined by Company policy.nnEnsures that client care issues, changes and/or updates are documented and communicated tonnbranch staff, direct care staff, case management and family.nnReviews HRST and other documentation as required for DD cases, meeting with branch staff,nndirect care staff, family and DD support team as necessary to understand and advise on clinicalnnissues.nnProvides or oversees the clinical orientation of new employees, including initial competencynnevaluation and training of staff as needednnPerforms written Annual Competency Evaluations of all direct care skilled personnel (RNs andnnLPNs) and non-skilled personnel (CNAs and PCAs). Participates in periodic conference with thennBranch Manager concerning direct care staff performance.nnAssesses and recommends solutions in areas of risk for clients, staff and organizationnnReviews and corrects documentation errors according to currently accepted standards ofnnmedical practice.nnEnsures appropriate follow through and completion of documentation for incidents of clinicalnnnature.nnProvides PRN visits as necessary per Company policy and/or as directed by supervisor.nnCompletes clinical audits as requested by supervisor.nnEnsures the compliant maintenance of TB testing supplies and proper disposal of syringes.nnQUALIFICATIONS:nn• Registered Nurse with valid license in state of GAnn• Demonstrate high level of organization and communicationnn• At least 24 months of prior nursing experience, preferably within a multiple location environmentnnKnowledge and Skills:nn• Excellent interpersonal, written, and verbal communication skillsnn• Strong analytical and problem-solving skills to assess situations and make accurate observationsnnin the workplacenn• Knowledge of health administration regulations and standards, health informationnnmanagement, and Medicaid, Waiver and Home health service linesnn• Advanced knowledge of Microsoft Office Suite and aptitude to learn new applicationsnn• Works accurately, efficiently, and independentlynn• Flexibility and ability to prioritize work with minimal supervision in a fast paced, high productivennenvironmentnn• Ability to maneuver multiple databases in the collection and submission of informationnn• Multi market or multi-site leadership experience (preferred)nnData Security and Privacy StatementnnAt Help at Home, we prioritize protecting your personal information during the hiring process.
