Help at Home also offers strong career path possibilities for those employees who are committed to growing within the organization.nnJoin us today and become a Help at Home Hero!nnA typical day for a RN Case Manager includes:nnConducting initial evaluation visits and regularly re-evaluating clients' nursing needs.nnCounseling clients and their families regarding nursing needs and related needs.nnCoordinating and creating service plans for clients following client assessments and admissions.nnAssisting and supervising field staff, ensuring successful implementation of new Medicaid Waiver programs.nnEnsuring quality care is delivered to our clients by creating, implementing, monitoring, and enforcing quality assurance policies and procedures.nnAssisting our branch teams with pre-service training and orientation, as well as in-service training for caregivers.nnAssisting with other duties as needed and as assigned by the Branch Manager.nnIn order to be eligible for this role, you should have:nnA current RN license in the state of servicesnnA valid driver's licensennAccess to reliable and insured transportationnnAbility to travel within the surrounding counties 2-3 days per week between the hours of 9:00am - 5:00pmnnAvailability to be On-Call for caseload-related crisesnnNursing experience preferrednnRN Case Managers must comply with state background screening requirements.
