Job Description:
- Provide direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Participate in continuing education and in-service programs to maintain knowledge and competency in nursing practice.
- Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and other members of the interdisciplinary team.
- Contribute to creating a developmentally and culturally relevant plan of resident care, reflecting nursing diagnosis and interventions for desired outcomes, involving the resident/family and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes, indicating the need for referrals to address all aspects of resident care.
- Collaborate with the resident/family and members of the interdisciplinary team within the scope of nursing practice, ensuring customer satisfaction.
- Participate in the mentoring/orientation of staff and provide leadership in the professional practice setting under the direction of the RN.
- Demonstrate knowledge and competency with attention to factors related to safety, cost, and impact in the planning and delivery of care.
- Anticipate occupational exposure to bloodborne pathogens with tasks potentially involving contact with blood, body fluids, or tissues.
