Key Responsibilities

n - Provide high-quality, compassionate care for patients in long-term care settings , focusing on geriatric nursing
n - Collaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.
n - Accurately document visits in an electronic medical record (EMR)
n - Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse .

Qualifications

Required Qualifications:
nn
n - Active Virginia Medical License and DEA License
n - Minimum one year of clinical nursing experience
n - Strong communication skills, self-motivated, adaptable, and a team player
n

Preferred Qualifications:
n
n - Nursing experience in ICU, Med/Surg, Emergency Medicine , or Hospitalist roles
n - Experience with care coordination and working in long-term care settings, especially geriatric nursing
