Dependable team player, detail-oriented, accurate, organized and flexible.CARE COORDINATOR RESPONSIBILITIES:Provide administrative backup including answering phones, in house scheduling, care coordination, pre-visit planning, demographics updates, fax management, visit check in/out, documentation management, and limited support for billing tasks, such as updating payment methods and providing receipts upon request. Assist with clinical support including visit intake, vitals, venipuncture, vaccine administration, EKG, Spirometry and other in-house procedures, as well as phone support, and proactive chronic care reminders in accordance with EHS standards. Assist with Rx Management, as requested in accordance to clinician's orders, including standing orders. Maintain ongoing task tracking and appropriate documentation to promote team awareness and ensure client safety. This tracking may require functional use of multiple platforms in accordance with procedure. Assist in obtaining current records, updating client's primary care chart with information on specialist consults, hospitalizations, ER visits and community organizations related to their health, allowing for a fully personalized experience with each care plan coordination handled by EHS, advising supervisor of any concerns promptly. Contact facilities and insurance companies to ensure prior approval requirements are met. Present necessary medical information as requested such as history, diagnosis and prognosis. Provide specific medical information to referral sources to maximize insurance reimbursement to the facility, clinician, and/or clients.Assist with care option research as required/requested (Ex: Identify & explain travel vaccination requirements).Review details and expectations about medical care with clients, using appropriate templates and approved verbiage, reminding clients of scheduled appointments via e-mail or phone based on personal preferences or time constraints. Assist in confirming care plans are addressed, appointments were kept, and all pre and post visit reports/notes/ results are processed appropriately, in a timely manner, advising supervisor of any concerns promptly. Seek to understand how to become a healthcare system navigator to become the point of contact for clients and families, allowing clients and families to have a direct source for asking questions and raising concerns. Establish and maintain positive relationships with identified service facilities, and all referral sources. Ensure compliance with all health care regulations, including HIPAA and OSHA. Operate in accordance to CLIA regulations & standards for in-house testing. Demonstrate a friendly, professional manner with clients, clinicians, peers, vendors, and associates. Maintain office and treatment room aseptic status with consideration for aesthetics and comfort. Perform other tasks as requested by Clinicians, Executive Coordinators, or Team Leads.
