We are seeking a dedicated Case Manager RN or Licensed Clinical Social Worker (LCSW) with strong hospital case management experience to join our team. About the Role

Shift: Monday – Friday, Day 5x8-Hour (08:30 - 17:00)

Contract Duration: 13 weeks

Responsibilities

• Provide hospital-based case management services.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Ensure compliance with hospital policies and procedures.
• Deliver compassionate and patient-centered care.

Qualifications

Experience: Minimum of 2 years as a Case Management RN, OR Strong hospital case management experience as an LCSW.

Certification: Current BLS certification required.

Licensure: Must hold a valid DC license at the time of submission.

Vaccination Requirements: Flu vaccine required (no exemptions). COVID-19 vaccine not required.
