Position Overview:
Pride Health is seeking a compassionate and skilled Social Worker to join our healthcare team in Atlanta, Georgia. This is a full-time, 13-week contract position offering guaranteed hours. The ideal candidate will have a strong background in hospital-based care coordination and social services, eager to make a difference in patient outcomes and provide high-quality support to patients and their families.
Key Responsibilities:
- Care Coordination: Collaborate with patients, families, and the healthcare team to manage and coordinate care.
- Case Management: Oversee patient care plans and ensure timely services and transitions.
- Social Support: Provide emotional and psychosocial support to patients and families.
- Utilization Review: Assess and ensure the appropriateness of medical treatments.
- Documentation: Maintain accurate patient records and comply with hospital policies.
- Collaboration: Work with medical staff to develop treatment and discharge plans.
- Patient Education: Educate patients and families about resources and care processes.
- Compliance: Follow hospital policies and ensure patient confidentiality.
Qualifications:
- Education: Master of Social Work (MSW) or a Bachelor's degree in a related field with a strong clinical background.
- Experience: Minimum of 2 years of hospital-based experience in case management, utilization review, or social services.
- Certifications: BLS (Basic Life Support) certification required & LCSW (Licensed Clinical Social Worker) in the state of Georgia is required.
- Skills: Strong interpersonal communication skills, the ability to manage multiple tasks, and proficiency in typing (approximately 30 words per minute).
- Other Requirements: Compassionate and patient-centered approach with the ability to handle sensitive situations effectively.
