*Key Responsibilities*:
* *Patient Assessment*: Perform thorough evaluations of patients' physical health, mobility, strength, and functional needs in their home environment.
* *Individualized Treatment Plans*: Develop, implement, and update personalized therapy plans tailored to meet patients’ specific goals and needs.
* *Therapeutic Interventions*: Administer hands-on therapeutic techniques, exercises, and assistive modalities to improve mobility, strength, and independence.
* *Patient Education*: Educate patients and family members on home exercises, safety, and techniques to prevent injury and enhance overall well-being.
* *Documentation*: Maintain accurate and timely clinical documentation of patient progress, treatment plans, and outcomes in compliance with company policies and regulatory guidelines.
* *Collaboration*: Work closely with other healthcare providers, including physicians, nurses, and caregivers, to ensure continuity of care and achieve optimal outcomes for patients.
* *Progress Monitoring*: Regularly monitor and evaluate the effectiveness of the therapy program, adjusting interventions as necessary to achieve patient goals.
* *Adherence to Policies*: Follow company policies, procedures, and infection control protocols while providing high-quality care.

Pay: $65.00 - $70.00 per hour
