*Overview*
We are seeking a skilled and compassionate Physical Therapist to join our home health team, focused on providing personalized care to adult patients in their homes. The ideal candidate will have a passion for helping patients regain their mobility and independence, possess a strong knowledge of anatomy and medical terminology, and be dedicated to delivering high-quality patient-centered care. This role offers the unique opportunity to work in patients' homes, providing one-on-one care tailored to their individual needs.

*Duties*
* Conduct in-home assessments and develop individualized treatment plans for adult patients.
* Implement therapeutic exercises and interventions to aid in patient recovery and improve mobility and independence.
* Monitor patient progress regularly and modify treatment plans as necessary to achieve optimal outcomes.
* Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive, holistic care.
* Provide education and guidance to patients and their families on home exercise programs, injury prevention, and strategies to maintain physical health.
* Utilize a variety of therapeutic modalities and techniques to address pain management, strength training, and functional mobility.
* Ensure proper documentation of patient care, progress notes, and treatment plans in compliance with regulatory and company standards.

*Qualifications*
* Degree in Physical Therapy from an accredited program.
* Valid state licensure as a Physical Therapist.

Job Types: Full-time, Contract

Pay: $65.00 - $70.00 per hour
