Coverage Needed

• As soon as credentialed - ongoing
• (3) three month duration, with probability of extension

Schedule

• Monday - Friday, 8a-5p (12-1 lunch)
  • would consider 4 days per week
• 36 patient care hours, 4 hours of admin built into schedule

Clinical Details

• Outpatient clinic
• Family Practice - pediatric through geriatric (Internal Medicine is also acceptable)
• High elderly population with multiple comorbidities
• 16+ patients per day, concentration on chronic disease management

Requirements

• Active NH License - full payer enrollment required
• Must have recent experience in outpatient primary care and comfort working autonomously
• Well experienced in chronic disease management and able to have difficult conversations with patients regarding medication management and patient education
• Board Certified
• Years of work experience: 2 years

• Supported by practice manager, nursing staff, and designated MA
