Job Summary:
As a Licensed Practical Nurse (LPN), you’ll provide compassionate, high-quality care tailored to our adult and geriatric residents' specific needs. You’ll ensure every resident receives individualized care in alignment with their personalized care plan.
Key Responsibilities:
Deliver exceptional care to residents, addressing physical, emotional, and mental needs.
Administer medications, treatments, and services with precision and documentation accuracy.
Participate in interdisciplinary care planning, facility meetings, and quality improvement initiatives.
Supervise nursing and support staff, fostering a positive and efficient working environment.
Respond to medical changes, family concerns, and ensure communication with care providers.
Ensure HIPAA compliance and uphold resident rights and safety standards.
Preferred Skills & Qualifications:
Expertise in managing care for patients with therapeutic, neuro, dementia, dialysis, wound care, or end-of-life needs.
Strong understanding of infection control, behavioral interventions, and emergency response.
Exceptional communication and teamwork skills, with a proactive and optimistic attitude.
