*What is Required?*
* Positive attitude
* Dedication to quality patient care
* Strong communication skills
* Reliable transportation to perform job duties
* RN license is required
* Hospice experience preferred

*Responsibilities*
* Provide comprehensive patient care for hospice patients, ensuring comfort and dignity in their final stages of life.
* Conduct thorough patient assessments to develop individualized care plans.
* Collaborate with interdisciplinary teams to coordinate services and support for patients and families.
* Educate patients and families on hospice care options and available resources.
* Adhere to HIPAA regulations and maintain accurate medical records.
* Communicate effectively with healthcare providers to ensure continuity of care.
