Your Responsibilities



As a Hospice RN Case Manager, you will:


* Provide patient care across the continuum, managing hospice services with empathy and expertise.
* Assess, plan, implement, and evaluate individualized care plans for patients.
* Support patients and families through end-of-life care with compassion and dignity.
* Collaborate with interdisciplinary teams to ensure seamless care delivery.
* Maintain accurate documentation and manage multiple priorities effectively.



What You Bring


* Education: Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate Nursing Program.
* Licensure: Current Registered Nurse (RN) license in [Insert State].
* Experience:
* Minimum of 1 year in a clinical care setting.
* Hospice or home health experience (preferred).
* Skills: Strong nursing skills, ability to manage priorities, and dedication to patient-centered care.
* Transportation: Reliable transportation to perform job responsibilities.
