POSITION SUMMARY – Social Worker

POSITION DUTIES – Hospice admission assessment / bereavement assessment / development of plan of care / follow up of family & client social needs / may assist with social needs of home health clients. Will float between Dickinson & Williston.

MINIMUM REQUIRED QUALIFICATIONS – BLS; ND Social Worker License & MUST hold one of the following: LMSW-AP, LCSW, CHP-SW; 1+ year home health/hospice social worker experience.

PREFERRED QUALIFICATIONS – HomeCare HomeBase experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Days - Mon-Fri 8-5 / 40hrs/wk

SYSTEMS – Home Care Home Base

START DATE – ASAP
