Social Worker

POSITION SUMMARY – Social Worker

POSITION DUTIES –

Provides coverage as needed at any of our facilities.
Provides comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and their families.
Addresses social determinants of health and works collaboratively with the multidisciplinary team.

MINIMUM REQUIRED QUALIFICATIONS – LCSW or LMSW (NY); BLS; Post-master's experience, preferably in a hospital or healthcare environment.

PREFERRED QUALIFICATIONS – 3 years in a hospital and/or healthcare setting

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 5 8hr days

START DATE – ASAP
