    Job Requirements
  • BLS certification required
  • License/Certificate Verification required within 30 days of start date
  • Fit Test required within 365 days of start date
  • MGB Medication Exam passing score required within 365 days
  • COVID-19 Vaccine required prior to start
  • MMR vaccination documentation required
  • TB Test documentation required within 3 months of start date
  • Health Statement required within 365 days of start date
  • MGB Criminal Background Check required within 45 days prior to start date
  • MGB Massachusetts CORI required within 30 days prior to start date
  • MGB Resume required for submission
  • MGB Skills Checklist required for submission
  • MGB Confirmation of Assignment/POI Form required upon acceptance of assignment
    Additional Details
  • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity required
  • Vaccine documentation for Hepatitis B preferred
  • Influenza Vaccine required during flu season
  • Documentation of a negative QuantiFERON TB Test or T-spot required if TB skin test is positive
  • Attestation for Fit Test Record form required
