Travel Nurse RN - Case Management
Position Overview
We are seeking an experienced Travel Nurse RN to fill a 13-week case management position in Grand Junction, CO. The ideal candidate will have a strong background in nursing and excellent communication skills.
Job Summary
This role is responsible for coordinating the care of patients with complex medical needs. The successful candidate will work closely with healthcare teams to develop and implement effective care plans, ensuring seamless transitions and optimal patient outcomes.
Key Responsibilities:
* Coordinate patient care services across multiple healthcare settings
* Develop and implement individualized care plans tailored to patient needs
* Collaborate with interdisciplinary teams to ensure comprehensive care delivery
* Communicate effectively with patients, families, and healthcare providers
* Maintain accurate and timely documentation of patient information
Requirements:
* 2+ years of experience as a Registered Nurse (RN)
* Strong background in case management or related field
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Basic Life Support (BCLS/BLS) certification required
