Responsibilities

  • Responsible for providing case management services to patients and families. Primary duties include discharge planning and transitions of care.
  • The case manager demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes.
  • Monitors physician documentation and works closely with the medical staff to assign the patient to the appropriate level of care.
  • Responsible for the initial utilization review screen and the ongoing utilization review process through discharge.
  • Identifies patient discharge and transition of care needs and is ultimately responsible for the coordination of the discharge planning activities.
  • Works collaboratively with the patient and interdisciplinary team to develop a safe discharge plan that minimizes the risk of hospital readmissions.
  • Actively participates in quality improvement and evaluation processes

Qualifications

Job Requirements:

  • Graduate from an accredited nursing program
  • Current Oklahoma nurses license.
  • BLS for Healthcare Providers.
  • Minimum of 5 years of Medical Surgical Nursing.
  • Case Management experience.

Preferred Job Requirements:

  • Bachelors of Science in Nursing (BSN)
  • Certified Case Manager (CCM)
