Join us at Fedcap Inc. for an exciting career as a Service Coordinator.nnWage/Salary: $44K | Job Title: Service Coordinator | Location: Bronx, New YorknnFLSA Status: Non-Exempt | Position Type: Full-TimennPosition Summary:nnThe Service Coordinator acts as the primary contact for all Youth Pathways participants and is responsible for following the participant throughout their time in the program, following services post-program, and ensuring they are connected to and receiving all appropriate services. The candidate should have a background in case management and social services, and a willingness and desire to work with at-risk and vulnerable populations. They will reflect the Fedcap values of teamwork, collaboration, and innovation.nnThe Service Coordinator is responsible for acting as the main point person for participants currently or formerly enrolled in the Youth Pathways program. The Service Coordinator is responsible for providing services to each participant on his or her caseload, including ensuring a proper service plan that includes participants' short and long-term goals, the connection to proper resources including education, training, job readiness, soft skills, and essential needs such as food and housing, and proper follow up with participants after they have left the program due to enrollment in job search, education, or obtaining employment.nnEssential Job Functions:nnWork collaboratively with participants to determine the enrollment of services and activities based on the assessment of the participant’s employment and educational goals, preferred location of service, educational and skill levels, potential barriers, and the service coordinator's own observation.nnAddress any immediate barriers that may prevent the participant from enrolling in appropriate services or activities.nnContinually evaluate and advocate for the appropriate services necessary to benefit the participant toward his or her goal achievement.nnGuide participants to make appropriate, effective decisions in support of their goals.nnAdapt recommendations and case strategy based on current understanding of participants’ perspectives, backgrounds, needs, etc.nnParticipate in the development, implementation, and monitoring of the Service Plan.nnResponsible for scheduling and meeting with participants regularly to review progress.nnSchedule participants for internal activities while enrolled in the program.nnFollow-up with participants post-program to monitor progress towards goals and address any potential barriers.nnComplete all administrative work such as entering information, such as case notes, into various databases.nnEducation and Experience:nnBachelor’s degree in Human Services, Social Work, Education, or comparable work-related experience.nnAt least 2 years of experience in case management or service delivery.nnExperience and/or a demonstrated desire to work with the economically disadvantaged youth (18-24), homeless, justice-involved, and/or public assistance to recipient populations.nnQUALIFICATIONS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.nnEqual Opportunity Employer
